Advancing Dementia Care: A New OT Initial Assessment Tool

Presentation summary

Background:
Dementia is a significant public health concern in Australia, impacting nearly 10% people over 65 and 30% over 85. National audits indicate that the quality of care for people with dementia and their carers requires improvement. Many occupational therapists (OTs) report limited confidence and lack specialist training in dementia care, and there is currently no standardised, person-centred OT initial assessment for dementia. This project aimed to develop an initial assessment tool to support evidence-based dementia care.

Methods:
A comprehensive approach was undertaken, including a literature review, benchmarking with other health organisations, staff surveys, and a clinical documentation audit. An initial assessment template was designed in consultation with OTs and consumers. An accompanying user guide to facilitate implementation. The template was trialed by OTs in acute and subacute settings, with feedback gathered from both clinicians and people living with dementia and their carers. An education campaign supported the rollout of the new documentation template.

Discussion/Outcomes:
Pre-survey and audit results highlighted gaps in OT confidence and knowledge regarding dementia-specific initial assessment. The new template and user guide address these gaps by guiding information gathering and supporting consistent, person-centred care. The education campaign further enhanced clinical reasoning and understanding among OTs.

Conclusion:
No gold standard exists for dementia initial assessment in OT practice, and current guidelines lack direction on essential information to support discharge planning and interventions. The design and implementation of a tailored dementia initial assessment into clinical practice may enhance the delivery of patient-centered care for hospitalised patients with dementia.

Danielle Byrne and Stephanie Mackay are clinical leads at Monash Health, specialising in cognition and gerontology respectively. Both with extensive UK and Australian experience, they drive quality improvement in older adult care, focusing on frailty, dementia, cognitive impairment through research, service development, and workforce education.
